Under what circumstances are AD&D benefits available?
By
1. Your death or dismemberment must result from an Accident which occurs during your Certificate Period.
2. Your death or dismemberment must occur within sixty (60) days of the Accident.
3. The Injury giving rise to your Accidental Death or Accidental Dismemberment must be evidenced by a visible contusion or wound, except in the case of Accidental drowning.
4. Your Accidental Death or Accidental Dismemberment must result directly and independently of all other causes, from an Accidental Injury which is unintended, unexpected and unforeseen, and the Injury must be the sole cause of your death.
